Notarized affidavit to the district superintendent by August 1 each year. The parent needs a high school diploma or GED. Keep a portfolio (reading list, work samples, time log: 180 days or 900/990 hours). Standardized tests in grades 3, 5 and 8. A qualified evaluator reviews the portfolio each year; certification due to the district by June 30.
